Group Health insurance

Your well-being is our priority. Our goal is to support your physical and mental health. By providing access to medical care, we help you stay healthy through regular check-ups and so that you can address health concerns as they arise.

We are currently offering a selection of three Anthem Blue Cross Blue Shield plans for the 2025-2026 year. Our company pays 100% of the employee medical premium of the employer selected plan (A) for eligible employees. Two higher cost plans (B & C) are also available. Employees who chose plans B & C will pay the difference in the premium. Eligible employees also have the option to add eligible dependents. Premiums for optional dependent coverage is paid by the employee. Employer contribution and selected plans are subject to change each year. If you already have health coverage through another plan, you can elect to waive coverage. Eligible employees waiving coverage because they do not need it may be provided alternative benefits such as bonus PTO days. Pricing for alternative plans will be distributed during open enrollment and upon new hire eligibilty. Price is age banded so is different for each employee. Please inquire at the office for further details.

Eligibility: Full time employees are eligible the 1st day of the month following completion of two (2) months of service. Employees regularly working a minimum of 24 hours a week are considered full time for eligibility purposes.

Open Enrollment: Open enrollment is overed during the month of November for enrollment date of December 1 each year. At this time eligible employees can enroll, change plan selection, add or drop dependents or waive coverage. Below we have provided the application form and plan descriptions for 12/1/2025 open enrollments valid through 11/30/26.

A - Anthem Gold Pathway EPO 1500/20%/8250 Rx Copay (Company will pay 100% of this plan)

B - Anthem Platinum Pathway EPO 500/10%/2500 Rx Copay (Employee pays difference in cost)

C -Anthem Platinum PPO 10/0%/2500 Rx Copay (Employee pays difference in cost)

How it works:

With Delta Dental Patient Direct, you can select a dentist from the growing 500-provider Patient Direct network. It’s easy to enroll and use. Simply follow these steps:

  1. Use the Find a Dentist link to see if there’s a dentist near you. Make sure to select Delta Dental Patient Direct in the Plan dropdown menu.
  2. Sign up through your employer’s group benefits program to access discounts from 15–70% on routine preventive and restorative dental care.
  3. Visit a network provider convenient to you. Know the fees you will have to pay with the Delta Dental Patient Direct Fee Schedule (NEW Direct Fee Schedule effective Nov. 1, 2025). No insurance claims to worry about — all fees are due at time of service.
